Checklist for ship to ship transfer or STS operation.

Discharging Ship’s Name:   Receiving Ship’s Name:    Date of Transfer:     Discharging Ship Checked Receiving Ship Checked Remarks   1.  The ISGOTT  Ship/Shore Safety Check List as been satisfactorily completed?         2.      Procedures for transfer of personnel have been agreed ?         3.      Is the gangway(if used) is in good position and well secured?         4.      Is an inter-ship communication system established?         5.  Are emergency signals and shutdown procedures agreed?         6.  Is an engine room watch maintained throughout transfer and is the main engine ready for immediate use?         7.    Are fire axes or suitable cutting equipment in position at fore and aft mooring stations?         8.  Is a bridge watch and/or an anchor watch established?         9.  Officers in charge of the cargo transfer on both ships are identified and posted ?         10. Is a deck watch established to pay  particular attention to moorings, fenders, hoses, manifold observation and cargo pump controls?         11. Is the initial cargo transfer rate agreed with other ship?         12. Is the maximum cargo transfer rate agreed   with other ship?         13. Is the topping-off rate agreed with other ship?         14.  Are the cargo hoses well supported ?         15. Are tools required for rapid disconnection  located at the cargo manifold?         16. Details of the previous cargo of the receiving ship have been given to the discharging ship ?         17.
