Has the ship’s stability and distribution of cargo been calculated for the intended loading condition?         12. Where appropriate, has the Administration Approval been obtained?         13. Have means of communication, including emergency communication been established and agreed by ship and shore?   R     14. Have checkpoints been agreed by the ship and shore at which quantity of cargo handled can be verified and compliance with cargo plan confirmed?   R     15. Have the hull stresses been calculated to confirm that they do not exceed the maximum allowable at any point during loading / discharging?   R     15. Have all ballasting and deballasting systems been checked? Does the vessel have adequate pumping capacity to meet the requirements of the cargo plan?   R     16. Has the need to trim cargo in holds been discussed and agreed?         17. Has an emergency procedure been agreed between the ship and terminal to stop cargo work?      

NOTE:          
  1. The safety of operations requires that all questions should be answered by both parties in affirmative. Where any check is “Not Applicable”, it  shall be marked as “n.a”.
  2. Entries regarding draft and cargo information  indicated in S.Nos 6 and 7 shall be made in the appropriate space given after the question.
  3. The presence of letter R in the column ‘Code’ indicates items to be re-checked at specified intervals.
DECLARATION

I have checked, where appropriate jointly with  shore representative, the items on this check list and have satisfied myself that the entries made here are correct to the best of my knowledge and arrangements have been made to carry out repetitive checks as necessary on items marked ‘R’ in the column ‘Code’.
