1. General (Annual and Intermediate Surveys)
Confirm no modifications have been made to the ship or equipment which would affect the
1.1 class (any modifications are to be reported). __
1.2 Confirm that all periodical surveys required for boilers and other pressure vessels are up to date. __
1.3 Confirm the CSM and CSH cycles are up to date in accordance with the current survey status. __
1.4 Confirm that the Periodical Survey of the automation and/or remote controls for the main propulsion plant are not overdue
1.5 Confirm that the Periodical Survey of the inert gas system is not overdue.
1.6 Confirm the Classification Certificate now endorsed for Annual Survey .
1.7 Confirm the Classification Certificate now endorsed for Intermediate Survey __
2. Documentation (Annual and Intermediate Surveys)
2.1 List the following certificate with their annual or intermediate survey date as well as expiry date :
(a) Cargo Ship Safety Construction Certificate
(b) Cargo Ship Safety Equipment Certificate
(c) Cargo Ship Safety Radio Certificate
(d) International Load Line Certificate
(e) International Oil Pollution Prevention Certificate
(f) International Safety Management Certification
(g) Cargo Ship Safety Certificate (where applicable)
2.2 The log entries record that the following have been checked or tested and found satisfactory within twelve (12) hours before departure from any port.
(a) Steering gear __
(The tests included, where applicable, the operation of the main steering gear, the auxiliary steering gear, remote control systems, bridge steering position, emergency power supply, rudder angle indicators, steering gear system power unit)
(b)Communications system bridge to steering gear compartment __
(c) Full movement of the rudder __
(d) Visual inspection of steering linkage __
2.3 The log entries record that emergency steering drills have been carried out every three (3) months. __
2.4 The change-over procedure diagrams of steering gear are posted. __
2.5 All officers are reportedly familiar with steering gear change-over procedures. __
2.6 The approved stability/loading information is on board. __
2.7 For ships assigned timber loadlines the approved timber deck cargo loading and lashing plan is on board. __
2.8 The required documentation for oil tankers (including combination carriers) and dry bulk cargo ships (bulk carriers) is on
board.
3. Hull Survey (Annual and Intermediate Surveys)
3.1 The condition of the hull and its closing appliances is satisfactory as far as could be seen, The following items should be
included as applicable(see also Appendix 1 for shell doors):
3.1.1 Hatchways; weather decks, casings, fiddley openings, skylights, flush deck scuttles, deckhouses and companionways; superstructures; windows; side scuttles; and deadlights; chutes and other openings; scuppers; sanitary discharges and valves; guard rails and bulwarks; freeing ports; gangways and lifelines; and permanent fittings for timber deck cargoes.
3.1.2 Vents and air pipes. Where appropriate vents and air pipe condition should be verified (Annual and Intermediate Surveys) __
3. 2 The condition of the anchoring and mooring equipment is satisfactory, as far as could be seen. __
The watertight doors in the watertight bulkheads 3.3 have been examined, operationally tested (remotely and locally) and
found satisfactory. __
3.4 The condition of the watertight bulkhead penetrations is satisfactory, as far as could be seen. __
3.5 The structural fire protection arrangements remain unchanged (Alterations are to be reported). __
3.6 The manual and/or automatic fire doors have been operationally tested and found satisfactory. __
3.7 The operation of the loading instrument has been verified. __
3.8 The freeboard marks have been verified. __
3.9 Hatch covers, coamings and gaskets have been checked and mechanically operated hatch covers tested and found weathertight.
Salt water ballast tanks
The tanks where protective coating was not applied at construction or has been reported in POOR condition and not
3.10 Repaired have been examined and found satisfactory. (report tanks) __
Note: Those tanks which may require to be internally examined at annual survey are indicated in the Survey Status.
Dry bulk cargo ships (bulk carriers)
(a) (1) Close-up survey of cargo hold steel hatch covers, coamings and stiffeners. __
(b) For ships less than 10 years old
(1) An overall survey of the forward cargo hold and an aft cargo hold (annual only) __
3.11 (c) For ships between 10 and 15 years old
(1) Overall survey of all cargo holds (annual and intermediate) __
(2) Close-up survey of at least 25% of the cargo hold side shell frames, their lower end attachments and adjacent shell plating in a forward cargo hold (annual only) __
(d) For ships greater than 15 years old†
(1) Overall survey of all cargo holds (annual only) __
(2) Close-up survey of at least 25% of the cargo hold side shell frames, their lower end attachments and adjacent shell plating in a forward cargo hold and one other selected cargo hold (annual only)
(e) The close-up survey has been extended to include all other cargo holds because of the need for remedial action (details to be reported). Any necessary thickness measurement is to be reported. _
3.12 Oil tankers including combination carriers, dry bulk cargo ships (bulk carriers) and chemical tankers
(a) Close-up survey and thickness measurement of areas of substantial corrosion identified in the Survey Status or the
Executive Summary carried out satisfactorily (report area(s) and thicknesses)
(b) All ballast tanks adjacent to a cargo tank with any means of heating have been examined and found satisfactory. Any
necessary thickness measurement is to be reported.
3.13 All ships assigned the special features notation “certified container securing arrangements”. The condition of the securing arrangements is satisfactory as far as could be seen. __
3.14 General dry cargo ships)
(a) (1) Close-up survey of cargo hold steel hatch covers, coaming and stiffeners.
(b) For ships between 10 and 15 years old†
(1) Overall survey of one forward and one after cargo hold and their associated tween deck spaces (annual only) __
(c) For ships greater that 15 years old†
(1) Overall survey of all cargo holds and tween deck spaces (annual only). __
(2) Close-up survey of at least 25% of all shell frames, including their end attachments and adjacent shell plating in a forward lower cargo hold and one other selected lower cargo hold (annual only).
(d) The close up survey has been extended to include all other cargo holds because of the need for remedial action (details to be reported). Any necessary thickness measurement is to be reported.
4. Machinery and Electrical Installation Survey (Annual and Intermediate Surveys)
4.1 The machinery and boiler spaces and essential machinery have been generally examined and found satisfactory. __
4.2 The emergency escape routes from the machinery and boiler spaces are free of obstruction. __
4.3 The machinery and boiler spaces are free of all visible fire and explosion hazards. __
Piping systems containing fuel oil, lubricating oil or other flammable oils where fitted have been examined and operated as far as practicable, special attention being paid to tightness, fire precaution arrangements, flexible hoses and sounding
4.3.1 Arrangements and found satisfactory. __
The main and auxiliary steering arrangements, including 4.4 their associated equipment and control system, have been examined, operated and found satisfactory. __
4.5 All the means of communication between navigating bridge, machinery control and alternative steering positions have been
tested and found satisfactory. __
4.6 The bilge pumping systems, including bilge wells, extended spindles, pumps and level alarms where fitted, have been
examined and operated as far as practicable and all found satisfactory. __
4.6.1 Non-metallic expansion joints in piping systems which penetrate the hull and are fitted below the deepest loaded
waterline have been examined and found satisfactory. __
4.7 An external examination of boilers, pressure vessels, including safety devices , foundations, controls, relieving gear, insulation, gauges and piping has been carried out as far as practicable, and found satisfactory. __
4.8 The electrical equipment and cabling forming the main and emergency electrical installations have been generally examined under operating conditions as far as practicable. Bonding straps for the control of static electricity and earthing arrangements as fitted have also been examined. __
4.9 The satisfactory operation of the main and emergency sources of power and electrical services essential for safety in an emergency mode have been verified. (Where the sources of power are automatically controlled they have been tested in the automatic mode.) __
4.10 A general examination of the following, where applicable, has been carried out and operation considered satisfactory:
(a) Automation equipment __
(b) UMS CCS ICC or IP system (indicate those appropriate with X) __
(c) DP ( ) and PCR (enter detail of notation and indicate with X if PCR is appropriate) __
The DP System is also to be tested under operating conditions to an approved test schedule
4.11 An examination of the electrical installation in areas which may contain flammable gas or vapour and/or combustible dust,
it has verified that it is in good condition and has been properly maintained
4.12 Confirm that the Machinery Planned Maintenance audit has been completed as applicable , see check list 2100PMS __
4.13 Confirm that the Screwshaft Condition Monitoring and Turbine Condition Monitoring audits have been completed as
Applicable
5. Statutory Survey Related Items (Annual and Intermediate Services)
5.1 The fire control plan and duplicate have been examined and are properly posted. __
5.2 All fire and/or smoke detection and alarm systems have been examined and tested, as far as practicable, and found
satisfactory. __
5.3 An operative test of the fire main system and each fire pump, including the emergency fire pump, has been carried out. separately, to demonstrate that the two required jets of water could be provided simultaneously from different hydrants. __
5.4 All fire hoses, nozzles, applicators and spanners are situated at their respective stations and in satisfactory condition. __
5.5 The fixed fire fighting system controls, piping, instructions and markings are properly maintained and serviced. __

5.6 All semi-portable and portable extinguishers are fully charged, in their stowed position and with valid service dates. __
5.7 The remote controls for stopping fans and machinery and shutting off fuel supplies in machinery spaces and where fitted, the remote controls for stopping fans in accommodation spaces and means of cutting off power to the galley are in working order. __
5.8 The closing arrangements of ventilators, annular spaces, skylights, doorways and tunnel where applicable are satisfactory. __

5.9 The fireman’s outfits are complete and in satisfactory condition.
5.10 Where Lloyd’s Register is authorised to carry out the Periodic Load Line Inspection, Load Line Annual Survey and/or SAFCON Annual or Intermediate1 Survey on behalf of the National Authority provided the AS/ITSS is satisfactorily completed as appropriate in accordance with the checklist requirements then the Load Line,
SAFCON survey dates may be recommended and the certificates endorsed accordingly provided in the case of the SAFCON the following is also dealt with:
a) manning requirements and certificates (SOLAS CH V – Reg 13) __
b) manoeuvring booklet with information displayed on bridge (SOLAS CH II – 1 Reg 28 Res A601 (15)) __
c) damage control plans for ships built 1/2/92 and after (SOLAS CH II – 1 Reg 23.1) __
5.11 Confirmation that the following is satisfactory:
a) structural fire protection and fire doors including remote stopping of ventilation systems (SOLAS CH II – 2)
b) ventilation of machinery spaces (SOLAS CH II –1 Reg 35) __
c) noise protection in machinery spaces (SOLAS CH II – 1 Reg 36) __
d) engineer’s alarm is audible in the engineer’s accommodation area (SOLAS CH II – 1 Reg 38) __
e) domestic gaseous fuel arrangements (SOLAS CH II – 2 Reg 51) __
f) first start arrangements (SOLAS CH II – 1 Reg 26) __
g) arrangements for escape from working spaces (SOLAS CH II – 2 Reg 45)
Oil Tankers, including Combination Carriers (Annual and Intermediate Surveys)- in addition above sections
Weather deck
6.1 Cargo tank openings including gaskets, covers, coamings, P/V valves and flame screens are all satisfactory as far as could be seen.
6.1.1 A portable instrument for measuring flammable vapour concentrations with sufficient spares and means of calibration is
satisfactory.
6.2 Flame screens on vents at all bunker, oily ballast and oily slop tanks and void spaces are satisfactory as far as could be seen. __
6. 3 An examination of cargo, crude oil washing, bunker, ballast and vent piping systems including vent masts and headers has
been carried out and all found in satisfactory condition.
6.3.1 An examination of cargo tank venting arrangements and secondary means of venting or over/ under pressure alarms, if
fitted, has been found operational and satisfactory.
6.4 The condition of electrical equipment in dangerous zones is satisfactory as far as could be ascertained. __
Cargo pump room
6.5 Confirm that no potential sources of ignition (in or near the cargo pump room) such as loose gear, excessive product in
bilges, excessive vapours, combustible materials, etc. have been observed and if found then eliminated.

6.6 The access ladders are in satisfactory condition. __
6.7 All electrical equipment is in satisfactory condition as far as could be ascertained. __
6.8 The pump room bulkheads are free of signs of oil leakage or fractures. __
6.9 The sealing arrangements of bulkhead penetrations are satisfactory. __
6.10 An external examination of piping systems has been carried out and all found in a satisfactory condition. __
6.11 The cargo, bilge, ballast and stripping pumps examined as far as practicable and found satisfactory for:
(a) excessive gland seal leakage. __
(b) operation of electrical and mechanical remote operating and shutdown devices. __
(c) integrity of pump room bilge system and pump foundations. __
6.12 The pump room ventilation system is operational, and the ducting intact, the dampers are operable and the screens are clean.
6.13 As far as could be seen, the installed pressure gauges on cargo discharge lines and level indicator systems are operational. __
Inert gas systems (where fitted)
6.14 From external examination, all components and piping found free of signs of corrosion or gas/effluent leakage. __
6.15 Both inert gas blowers are operational. __
6.16 The scrubber room ventilation system is operational. __
6.17 The deck water seal filling and draining system is operational and without evidence of water carry-over. __
6.18 The non-return valve is operational. __
6.19 The operation of all remotely operated or automatically controlled valves, in particular the flue gas isolating valve(s) found
satisfactory.
6.20 The interlocking feature of the soot blowers checked and found satisfactory. __
6.21 The gas pressure regulating valve automatically closes when the inert gas blowers are secured
